Initial Comments:
Based on the findings of an unannounced on-site incident investigation survey conducted on June 4, 2026, Traditional Home Care, was found not to be in compliance with the requirements of 28 Pa. Code, Health Facilities, Part IV, Chapter 51, Subpart A.

(f) If a health care facility is
aware of a situation or the occurrence
of an event at the facility which
could seriously compromise quality
assurance or patient safety, the
facility shall immediately notify the
Department in writing.
The notification shall include
sufficient detail and information to
alert the Department as to the reason
for its occurrence and the steps which
the health care facility shall take to
rectify the situation.
Observations:
Based on review of Event Reporting System website, agency documentation and interview with facility care coordinator (employee # 1), it was determined that agency failed to notify the Department of Health Event Reporting System as defined in 28 Pa Code Chapter 51 within 24 hours of occurrence or discover for event which seriously compromised quality assurance and patient safety for one (1) of one (1) event requiring submission. (Event #1)
Findings include:
Event 1: Agency employee left early from her shift causing the consumer to attempt to cook for himself and during this, the oxygen cord got caught on the stove dial, causing the consumer to get severely burned and eventually passing away at the hospital due to developing pneumonia from his injuries.
Surveyor asked employee # 1 if the agency submitted a ERS report to the Department of Health regarding this incident, employee # 1 replied, " I am not sure, this was my first day on the job. " No event report was ever submitted by the agency to the Department of Health.
An interview with employee # 1 conducted on June 4, 2026, at approximately 11:00 a.m. confirmed the above findings.
