Initial Comments:
Based on the findings of an off-site follow-up and complaint investigation survey completed 12/16/2025 and 12/17/2025, Extensive Care LLC had corrected the deficiency cited under PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart H, Chapter 611, Home Care Agencies and Home Care. The deficiency was cited as a result of a Licensure Survey completed 10/30/2025.
