Initial Comments:
An off-site, state licensure follow-up survey conducted on September 12 and September 19, 2025 found that Icare Elder Services, LLC had corrected the deficiencies cited under PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries. The deficiencies were cited as a result of a state re-license survey completed on July 18, 2025.
