Initial Comments:An offsite follow-up survey completed February 24, 2025 found that Grand View Home Care had corrected the deficiencies cited under the requirements of 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The deficiencies were cited as a result of a federal recertification survey completed on January 10, 2025.

Code, Part IV, Health Facilities, Subpart G. Chapter 601. The deficiencies were cited as a result of an onsite state relicensure survey completed January 10, 2025.
