Initial Comments:
An on-site follow up survey conducted on March 24, 2025, found that Epitome Home Care Inc had corrected the following deficiency cited under 28 Pa. Code, Health Facilities, Part IV, Chapter 51, Subpart A. The deficiency was cited as a result of a re-visit and compliant investigation survey completed on December 5, 2024.
