Based on an Initial Licensure completed on November 25, 2025, it was determined there were no deficiencies identified as related to the requirements of the Life Safety Code for a new ambulatory health care occupancy.
This Final Occupancy Survey was conducted for a new, one-story, Class C Ambulatory Surgery Center for ophthalmology surgery procedures.

This is a one-story, Type II (000), unprotected noncombustible structure, with a basement, which is fully sprinklered.
