Based on an Initial Licensure conducted on July 31, 2025, and completed on August 22, 2025, it was determined there were no deficiencies identified as related to the requirements of the Life Safety Code for a new Ambulatory health care occupancy.
This Final Occupancy Survey was conducted for Alterations on the second floor of a four-story building to create a new Class C, plastic surgery ambulatory surgery center.

This is a four-story, Type II (000), unprotected, non-combustible building, that is fully sprinklered.
Approved as a Class C Ambulatory Surgical Facility.
