Initial Comments:
Based on the findings of an unannounced state complaint investigation survey initiated March 30, 2026 and completed April 23, 2026, Guiding Hands Elder Care Services LLC, was found not to be in compliance with the requirements of 28 Pa. Code, Health Facilities, Part IV, Chapter 51, Subpart A.

Observations:
Based on observations (OBS) and interview with agency staff (EMP) the agency failed to provide at least sixty (60) days notification prior to the effective date of its intention to cease providing an existing service for two (2) of two (2) observatioms made. OBS #1, OBS #2.
Findings included:
OBS #1 completed on 4/1/26 at 3:00 PM revealed a location of: 441 East Market Street York, which is an old warehouse with multiple floors and businesses. Investigation of the building directory showed the Agency 's name listed as being located on the 2nd floor with a mailbox which listed agency as unit 5201. Further investigation found unit 5201 to be completely empty with no signs of an active business.
OBS #2, on 4/23/26 at approimately 2:45PM an onsite visit to 334 Penn St. Reading was completed to verify the operation of a reported office. Building closed with a black gate locked around store front. Signage on the Door read, "Lexi Bail Bonds" there was not additional signage to indicate any other business was operating at location. The building was empty, dark with no signs of anyone inside.
Multiple attempts were made to contact agency via telephone on 3/30, 3/31, 4/6 and 4/8/26 to verify agency operation voice messages left with no return calls received.
Interview with EMP (DCW #2) completed on 3/31/26 at 4:50 PM via telephone confirmed the above findings. " The agency basically ghosted us, didn't tell us anything, just disappeared off the face of the earth. "

(a) The consumer of home care services provided by a home care agency or through a home care registry shall have the following rights: (1) To be involved in the service planning process and to receive services with reasonable accommodation of individual needs and preferences, except where the health and safety of the direct care worker is at risk. (2) To receive at least 10 calendar days advance written notice of the intent of the home care agency or home care registry to terminate services. Less than 10 days advance written notice may be provided in the event the consumer has failed to pay for services, despite notice, and the consumer is more than 14 days in arrears, or if the health and welfare of the direct care worker is at risk.
Observations:

INT#2 with EMP (DCW # 3) completed on 4/8/26 at 3:24 PM via telephone also confirmed the above findings. "I' m a caregiver for (CF # 5) under the agency of Guiding Hands Home Health Care, we have not been paid and it's difficult to get in touch with any of the management in guiding hands. I say we because we are a number of caregivers who are experiencing problems of late". Caller stated that she has had no communication from the agency, she has tried calling the numbers that she had and hasn't received a response. She only had one (1) client with the agency and she worked thirty-seven (37) hours a week for them. Her hours are Friday from 7 PM-7 AM, Saturday 7 PM-7 AM and Sunday from 7 PM-8 AM.
