Initial Comments:
Based on the findings of an onsite unannounced Medicare recertification survey completed on June 8, 2026, Sharon Family Medicine Center Mercer was determined NOT to be in compliance with the following requirements of 42 CFR, Part 405, Subpart X and 42 CFR, Part 491.1 - 491.12, Subpart A, Conditions for Certification: Rural Health Clinics.
Onsite observation on 5/8/25 at approximately 9:00am - surveyor onsite at facility - per clinic manager "...this medical facility is not billing or operating as a rural health clinic as of January 2025... We are soley a family practice and no longer a rural health clinic. We are also relocating most likely at the end of the summer due to not being able to renew our lease. Meadville Medical Center acquired our practice back in January (2025) and should have sent notification about the change in status."
Phone call to 724-662-4155; phone number on file with the Pennsylvania Department of Health; on 5/29/25. The surveyor phoned facility requesting update on facility's current operations, spoke with facility manager "We are a family practice. are NOT operating as a Rural Health Clinic(RHC). actually got an eviction notice on 5/12/25 from Tenor stating we needed to vacate the premises within 30 days. acquired a new lease in the same building but a different suite. There is not a current facility operating as an RHC under Tenor Health Foundation." is unable to conduct an onsite recertification survey related to void of operations by Tenor Health Foundation.
On 12/1/25 a phone call conversation took place. Surveyor placed phone call to the Administration Staff at Sharon Regional Health System at 724-983-3911 that revealed: State surveyor spoke with Administration staff (ADM1) at Sharon Regional Health System "...we have a rural health clinic in Mercer. I believe it's not currently open and they are working on getting it opened. The best person to contact regarding this would be administrator (ADM2) at 724-866-7667." approximately 8:45am state surveyor attempt to phone administrator at phone number provided by ADM1 at Sharon Regional Health System. Surveyor spoke with ADM2 at approximately 8:50am. ADM2 states "... We will not be opening today. meant to get a new sign out to the facility with anticipation date of January 5, 2026 reopening. as Tenor have not actively operated as a rural health clinic, and have not provided services there since closure date of January 6, 2025." communications received from Sharon Regional Health System, Sharon Family Medicine Center Mercer Administrator (CRNP) as follows:

"...Sent: Monday, February 16, 2026 11:52 AM... Subject: Sharon Family Medicine Center Mercer- Continued with Delayed Opening... Good Morning, We are updating our opening date for our Mercer location due to renovations not being completed at this time. We are hopeful for 3/31/26 as an opening date. If there are any further updates as the new opening date approaches, I will be sure to contact everyone. If there are any further questions, please contact the Administration Staff at Sharon Regional Health System at 724-983-3911. (CRNP) Sharon Family Medicine Center Mercer, 551 Greenville Road Mercer, PA 16137..."
Email communications received from Sharon Regional Health System, Sharon Family Medicine Center Mercer Administrator (CRNP) as follows:
The following email sent to Pennsylvania Department of Health personnel on:June 2, 2026 11:44 AM; Subject:[External] Re: Sharon Family Medicine Center Mercer- Continued with Delayed Opening; ...At this time, this clinic is indefinitely closed and will not be opening. I will be posting a new sign. If there are any updates to this in the future, I will update the team..."on 6/8/26 at approximately 11:30 am revealed the facility doors were locked, there was a note on the door indicating "Sharon Family Medicine Center Mercer. We are currently closed due to ongoing building renovations. We anticipate opening June 1, 2026...". There was a notice taped to the entry door for "Electric Power Shut Off" from "Penn Power" electric company supplier stating "... Your electic account is past due. Under the rules established by the Public Utility Commission, we are notifying you that we intend to shut off electric service sometime on or after 8:00 am, on 5/18/26..." Surveyor is unable to conduct an onsite recertification survey related to void of operations by Tenor Health Foundation.
