Initial Comments:


Based on the findings of an onsite unannounced state re-licensure survey conducted on December 8, 2025, Interim Healthcare of Western Pennsylvania Inc., was found to be in compliance with the requirements of 28 Pa. Code, Health Facilities, Part IV, Chapter 51, Subpart A.

Based on the findings of an onsite home care agency state re-licensure survey conducted on December 8, 2025, Interim Healthcare of Western Pennsylvania was found not to be in compliance with the requirements of 28 Pa. Code, Health Facilities, Part IV, Chapter 611, Subpart H. Home Care Agencies and Home Care Registries.

611.51(a) LICENSURE
Hiring or Rostering Prerequisites

Name - Component - 00
Prior to hiring or rostering a direct care worker, the home care agency or home care registry shall: (1) Conduct a face-to-face interview with the individual. (2) Obtain not less than two satisfactory references for the individual. A satisfactory reference is a positive, verifiable reference, either verbal or written, from a former employer or other person not related to the individual that affirms the ability of the individual to provide home care services. (3) Require the individual to submit a criminal history report, in accordance with the requirements of § 611.52 (relating to criminal background checks), and a ChildLine verification, if applicable, in accordance with the requirements of § 611.53 (relating to child abuse clearance).

Observations:


Based on review of personnel files (PF) and interview with facility administrator the agency failed to ensure, prior to hiring, that two satisfactory references were obtained and verified for seven (7) of seven (7) files reviewed. PF #1 through PF#7.

Findings include.

Review of personnel files completed on 12/8/25 , between approximately 11:00AM and 1:30PM revealed the following:

PF#1 , Date of Hire (DOH): 6/13/25. contained incomplete documentation of two (2) satisfactory references being obtained.

PF#2, DOH: 8/5/24. contained incomplete documentation of two (2) satisfactory references being obtained.

PF#3, DOH: 2/20/23. contained incomplete documentation of two (2) satisfactory references being obtained.

PF#4, DOH: 10/15/25. contained incomplete documentation of two (2) satisfactory references being obtained.

PF#5, DOH: 2/13/23. contained incomplete documentation of two (2) satisfactory references being obtained.

PF#6, DOH: 1/9/25. contained incomplete documentation of two (2) satisfactory references being obtained.

PF#7, DOH: 5/26/24. contained incomplete documentation of two (2) satisfactory references being obtained.


12/8/25, at approximately 2:00PM, an interview conducted with facility administrator confirmed the above findings. "We started to use this "tracking" reference sheet form and did not get responses from some of the references. I thought as long as we tried it was ok."













Plan of Correction:

Policy Revision / Process Improvement:
1. Assign HR/Administrator responsibility to ensure two positive references are obtained and documented before hire date.
2. Implement new Reference Verification Form for all future hires.

Training:
1. Conduct staff training on hiring requirements and documentation standards.
2. Ensure all HR staff understand satisfactory references cannot be attempts, they must be verified positive responses.

Monitoring / QA:
1. QA audit of 100% of new hire personnel files monthly for six months to ensure compliance.
