Initial Comments:
An off-site follow-up survey conducted November 6, 2025, found that Moravia Health, had corrected the deficiencies cited under Title 28 Pa. Code, Part IV, Health Facilities, Part IV, Chapter 611, Subpart H: Home Care Agencies and Home Care Registries. The deficiencies were cited as a result of an on-site state re-licensure survey completed on January 2, 2026.
