Initial Comments:
Based on the findings of an onsite follow-up survey completed 12/16/2025, Aveanna Healthcare had corrected the deficiency cited under PA Act 54 of 1999, Prescribed Pediatric Extended Care Centers Act, 35 PS, Section 449.61. The deficiency was cited as a result of a State Relicensure survey completed on 09/24/2025.
