Initial Comments:
An offsite follow-up survey conducted on November 26, 2024, found that Guardian Hospice Care, Inc., had corrected the following deficiencies cited under 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiencies were cited as a result of a Medicare Initial certification and state licensure survey completed on November 8, 2024.
