Based on a Relicensure Survey completed on January 11, 2024, it was determined that Liberty Eye Surgery Center, Llc was not in compliance with the following requirements of the Life Safety Code for an existing Ambulatory health care occupancy.
This is a one-story, Type II (000), unprotected, non-combustible construction, which is fully sprinklered.

Portable Fire Extinguishers
Portable fire extinguishers are selected, installed, inspected, and maintained in accordance with NFPA 10, Standard for Portable Fire Extinguishers.
20.3.5.3, 21.3.5.3, 9.7.4.1, NFPA 10
Observations:
Based on observation and interview, it was determined the facility failed to maintain fire extinguisher inspections, affecting one fire extinguisher.
Findings include:
Observation made on January 11, 2024, at 9:00 am, revealed the electrical room fire extinguisher tag was missing its monthly quick-check inspections.
Exit interview with the Administrator and Director of Maintenance on January 11, 2024, at 9:40 am, confirmed the missing monthly quick checks.
Plan of Correction:Cintas Fire completed fire extinguisher inspection for all extinguishers in the building, including the electrical room, on 1/22/2024. Administrator will check all extinguishers monthly, leaving her initials on the monthly check tags provided by Cintas.
