Initial Comments:
An offsite survey completed on January 29, 2025 found that Heartland Hospice (Allentown) had corrected the deficiency cited under the requirements of 42 CFR, Part 418, Subparts A, C &; D, Conditions of Participation: Hospice Care. The deficiency was cited as a result of a state relicensure survey completed October 3, 2024 and a follow-up state relicensure survey completed on January 11, 2025.
