Initial Comments:
Per receipt of correspondence dated December 10, 2025, the provider agency has requested voluntary closure of the above location as an ICF/IID residence, to be effective November 20, 2025. The purpose of this action is to discontinue participation under 42 CFR, Part 483, Subpart I Regulations for Intermediate Care Facilities for Individuals with Intellectual Disabilities. The total number of beds currently certified under this location totals nine (9) ICF/IID certified beds.
All applicable materials have been forwarded to the Central Office of the Division of Intermediate Care Facilities for processing on December 11, 2025,
