Initial Comments:
Based on the findings of an offsite unannounced follow up survey completed on June 24, 2025 Diversified Health Care Inc. had corrected the deficiencies cited under PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries. The deficiencies were cited as a result of an unannounced re-licensure survey completed on May 8, 2025.
