Initial Comments:
An off-site, state licensure follow-up suruvey conducted on January 16, 17 and 22, 2025 found that Revolutionary Homecare had corrected the deficiency cited under 28 Pa. Code, Part IV, Health Facilities, and Subpart G. Chapter 601. The follow-up survey was conducted as a result of a state licensure follow-up survey completed on November 22, 2024.
