Initial Comments:
A follow-up survey conducted off-site on May 23, 2025, May 30, 2025, and June, 27, 2025, and on-site on June 16, 2025, found that Victory Place Home Health Care, LLC had corrected the following deficiencies cited under 28 Pa. Code, Part IV, Health Facilities, Subpart G. Chapter 601. The deficiencies were cited as a result of a re-licensure survey completed on March 6, 2025.
