Initial Comments:Based on the findings of an onsite unannounced home health agency Medicare recertification, state re-licensure and complaint investigation survey conducted February 24, 2025 through February 27, 2025 and concluded off-site March 17, 2025, Reliance Healthcare HomeHealth, was found to be in compliance with the requirements of 42 CFR, Part 484.22, Subpart B, Conditions of Participation: Home Health Agencies - Emergency Preparedness

Initial Comments:Based on the findings of an onsite unannounced home health agency Medicare recertification, state re-licensure and complaint investigation survey conducted February 24, 2025 through February 27, 2025 and concluded off-site March 17, 2025, Reliance Healthcare HomeHealth, was found to be in compliance with the requirements of 42 CFR, Part 484.22, Subpart B, Conditions of Participation: Home Health Agencies.
