Based on an Occupancy Survey completed on May 19, 2025, it was determined there were no deficiencies identified as related to the requirements of the Life Safety Code for an existing health care occupancy.

This occupancy inspection was conducted for alterations to enlarge the existing outpatient recovery area to accommodate 19 patient care stations and add a Class 2 CT room. This Phase Four inspection included: Patient Bays 15-17, Room 18, Soiled Utility 01-107D, Restroom 01-107C1, and Pre-action Closet 1111. Refer to Drawing Index Numbers: H-23-0549, H-23-0989, H-24-0131, and H-24-0729.

This is a five-story, Type II (222), fire resistive building, with a basement, that is partially sprinklered
