This report is the result of a full Medicare recertification survey conducted on March 24, 2026, at Advanced Surgery Center of Lancaster. It was determined the facility was in compliance with the requirements of 42 CFR, Title 42, Part 416 - Conditions for Coverage for Ambulatory Surgical Centers.

Initial comments:
This report is the result of a full State Licensure survey completed on March 24, 2026, at Advanced Surgery Center of Lancaster. It was determined the facility was not in compliance with the requirements of the Pennsylvania Department of Health ' s Rules and Regulations for Ambulatory Care Facilities, Annex A, Title 28, Part IV, Subparts A and F, Chapters 551-573, November 1999.






 Plan of Correction:


555.33 (d)(3) LICENSURE Anesthesia Policies and Procedures:State only Deficiency.
555.33 Anesthesia policies and procedures

(d) Anesthesia procedures shall provide at least the following:
(3) Prior to beginning the administration of anesthesia, the anesthetist shall check equipment to be used in administration of anesthetic agents. An anesthetic gas machine in anesthetising areas shall have a pin-index system.


Observations:

Based on review of facility policy, medical records (MR) and interview with staff (EMP), it was determined the facility failed to ensure prior to beginning the administration of anesthesia, that the anesthetist equipment was checked in 6 out of 20 medical records reviewed (MR2, MR6, MR7, MR14, MR15 and MR20).
Findings include:
Review of facility policy: "4.1 Anesthesia Responsibilities" with a last reviewed date of 03/01/2021 revealed "Anesthesia policies and procedures ... (d) Anesthesia procedures shall provide at least the following: ... (3) Prior to beginning the administration of anesthesia, the anesthetist shall check equipment to be used in administration of anesthetic agents."
Review of MR2 on March 24, 2026, revealed no documentation that the anesthetist checked the equipment to be used in administration of anesthetic agents prior to beginning the administration of anesthesia.
Review of MR6 on March 24, 2026, revealed no documentation that the anesthetist checked the equipment to be used in administration of anesthetic agents prior to beginning the administration of anesthesia.
Review of MR7 on March 24, 2026, revealed no documentation that the anesthetist checked the equipment to be used in administration of anesthetic agents prior to beginning the administration of anesthesia.
Review of MR14 on March 24, 2026, revealed no documentation that the anesthetist checked the equipment to be used in administration of anesthetic agents prior to beginning the administration of anesthesia.
Review of MR15 on March 24, 2026, revealed no documentation that the anesthetist checked the equipment to be used in administration of anesthetic agents prior to beginning the administration of anesthesia.
Review of MR20 on March 24, 2026, revealed no documentation that the anesthetist checked the equipment to be used in administration of anesthetic agents prior to beginning the administration of anesthesia.
Interview with EMP1, EMP2, and EMP3 on March 24, 2026, at approximately 1:00PM confirmed that there was no documentation that the anesthetist checked the equipment to be used in administration of anesthetic agents prior to beginning the administration of anesthesia for MR2, MR6, MR7, MR14, MR15 and MR20.

This has been updated and any cases scheduled after 3/25/2026 will have the updated form and all appropriate documentation for anesthesia.
