(A) A second full-scale exercise that is community-based or a facility based functional exercise; or

Based on a Medicare/Medicaid Recertification Survey completed on January 28, 2026, it was determined that Greenfield Healthcare and Rehabilitation was not in compliance with the following requirements of the Life Safety Code for an existing health care occupancy. Compliance with the National Fire Protection Association's Life Safety Code is required by 42 CFR 483.90(a).

This is a one-story, Type III (200), unprotected, ordinary building, with a basement, that is fully sprinklered.

NFPA 101 STANDARD Doors with Self-Closing Devices:This is a less serious (but not lowest level) deficiency and is isolated to the fewest number of residents, staff, or occurrences. This deficiency is one that results in minimal discomfort to the resident or has the potential (not yet realized) to negatively affect the resident's ability to achieve his/her highest functional status.

Doors in an exit passageway, stairway enclosure, or horizontal exit, smoke barrier, or hazardous area enclosure are self-closing and kept in the closed position, unless held open by a release device complying with 7.2.1.8.2 that automatically closes all such doors throughout the smoke compartment or entire facility upon activation of:
* Required manual fire alarm system; and
* Local smoke detectors designed to detect smoke passing through the opening or a required smoke detection system; and

Based on observation and interview, the facility failed to maintain doors with self-closing devices, affecting one of over five doors.

Findings include:

Observation on January 28, 2026, at 1:03 p.m., revealed the first floor, east wing, soiled utility room door had the self-closing device missing.

Interview with the maintenance supervisor on January 28, 2026, at 1:03 p.m., confirmed the missing closing device.





 Plan of Correction - To be completed: 02/24/2026

1. A self-closing device was added to the East wing, soiled utility room.
2. The other soiled utility doors have been inspected by Maintenance have a self-closing device.
3. Quality Assurance Committee will receive a monthly report from maintenance on status of building inspections. Monitored by the Administrator or designee.


NFPA 101 STANDARD Sprinkler System - Maintenance and Testing:This is a less serious (but not lowest level) deficiency and affects more than a limited number of residents, staff, or occurrences. This deficiency is one that results in minimal discomfort to the resident or has the potential (not yet realized) to negatively affect the resident's ability to achieve his/her highest functional status. This deficiency was not found to be throughout this facility.
Sprinkler System - Maintenance and Testing
Automatic sprinkler and standpipe systems are inspected, tested, and maintained in accordance with NFPA 25, Standard for the Inspection, Testing, and Maintaining of Water-based Fire Protection Systems. Records of system design, maintenance, inspection and testing are maintained in a secure location and readily available.

Based on observation and interview, the facility failed to maintain the sprinkler system for three of eight kitchen sprinkler heads.

Findings include:

Observation on January 28, 2026, at 1:50 p.m., revealed the first floor kitchen had three of eight sprinkler heads covered with debris, potentially causing a delay in sprinkler activation.

Interview with the maintenance supervisor on January 28, 2026, at 1:50 p.m., confirmed the sprinkler heads were dirty.





 Plan of Correction - To be completed: 02/24/2026

1. The 3 sprinkler heads in the kitchen have been cleaned.
2. The other sprinkler heads in the building have been visually inspected by Maintenance and cleaned when necessary. Sprinkler head inspection will be done monthly by Maintenance/designee.
3. Quality Assurance Committee will receive a monthly report from maintenance on status of building inspections. Reviewed by the Administrator.

NFPA 101 STANDARD Subdivision of Building Spaces - Smoke Compar:Least serious deficiency but was found to be widespread throughout the facility and/or has the potential to affect a large portion or all the residents. This deficiency has the potential for causing no more than a minor negative impact on the resident.

Smoke barriers shall be provided to form at least two smoke compartments on every sleeping floor with a 30 or more patient bed capacity. Size of compartments cannot exceed 22,500 square feet or a 200-foot travel distance from any point in the compartment to a door in the smoke barrier.

Observation on January 28, 2026, at 9:00 a.m., revealed the facility had incomplete smoke barriers throughout the building.

Interview with the maintenance supervisor on January 28, 2026, at 9:00 a.m., confirmed the facility had incomplete smoke barriers throughout the building.

1. Smoke barriers have been repaired throughout the building. Ceiling tiles have been replaced on the first floor.
2. Inspection of ceilings has been placed on a monthly TELS walkthrough by the maintenance department.
3. Quality Assurance Committee will receive a monthly report from maintenance on status of building inspections. Monitored by the Administrator.

Smoke barriers shall be constructed to a 1/2-hour fire resistance rating per 8.5. Smoke barriers shall be permitted to terminate at an atrium wall. Smoke dampers are not required in duct penetrations in fully ducted HVAC systems where an approved sprinkler system is installed for smoke compartments adjacent to the smoke barrier.

Based on observation and interview, the facility failed to maintain the smoke barrier on one of two building levels.

Findings include:

Observation on January 28, 2026, at 1:15 p.m., revealed the west side of the basement had multiple missing ceiling tiles.

Interview with the maintenance supervisor on January 28, 2026, at 1:15 p.m., confirmed the ceiling tile deficiency.

1. Smoke barriers have been repaired throughout the building. Ceiling tiles have been replaced on the first floor.

This deficiency is one that results in minimal discomfort to the resident or has the potential (not yet realized) to negatively affect the resident's ability to achieve his/her highest functional status.
Fire Drills
Fire drills include the transmission of a fire alarm signal and simulation of emergency fire conditions. Fire drills are held at expected and unexpected times under varying conditions, at least quarterly on each shift. The staff is familiar with procedures and is aware that drills are part of established routine. Where drills are conducted between 9:00 PM and 6:00 AM, a coded announcement may be used instead of audible alarms.

Based on document review and interview, the facility failed to meet fire drill requirements for four of twelve drills.

Findings include:

Document review on January 28, 2026, at 11:00 a.m., revealed fire drills for the third shift were conducted within the same hour slot for all of the last four quarters.

Interview with the maintenance supervisor on January 28, 2026, at 11:00 a.m., confirmed the fire drill deficiency at the time of the survey.





 Plan of Correction - To be completed: 02/24/2026

1. A fire drill has been held on 3rd shift at a random time, not between the hours of 5 and 6am.
2. Future fire drills will be randomized in timing. Maintenance Director will report on timing of drills after the fact to administrator if they are not present during the drill.
3. Quality Assurance Committee will review fire drill timing during monthly meeting. Monitored by Administrator or Designee.

NFPA 101 STANDARD Electrical Systems - Receptacles:Least serious deficiency but affects more than a limited number of residents, staff, or occurrences. This deficiency has the potential for causing no more than a minor negative impact on the resident but is not found to be throughout this facility.
Electrical Systems - Receptacles
Power receptacles have at least one, separate, highly dependable grounding pole capable of maintaining low-contact resistance with its mating plug. In pediatric locations, receptacles in patient rooms, bathrooms, play rooms, and activity rooms, other than nurseries, are listed tamper-resistant or employ a listed cover.
If used in patient care room, ground-fault circuit interrupters (GFCI) are listed.
6.3.2.2.6.2 (F), 6.3.2.2.4.2 (NFPA 99)
Observations:
Name: MAIN BUILDING 01 - Component: 01 - Tag: 0912

Based on observation and interview, the facility failed to maintain electrical receptacles for two of more than fifty receptacles.

Findings include:

Observation on January 28, 2026, between 1:05 p.m. and 1:45 p.m., revealed the facility failed to ensure ground fault circuit interrupter (GFCI) protection in the following areas:

1. (1:05 p.m.) First floor east wing nurse station watercooler;
2. (1:45 p.m.) First floor west wing nurse station watercooler.

Interview with the maintenance supervisor on January 28, 2026, at 1:45 p.m., confirmed the electrical outlet deficiencies.





 Plan of Correction - To be completed: 02/24/2026

1. Ground Fault circuit interrupter (GFCI) outlets have been installed behind both water coolers. 1 on east wing, 1 on west wing.
2. Other outlets near a water source have been inspected for need of a GFCI. No others were found to be missing.
3. Quality Assurance Committee will receive a report of building inspections from Maintenance during monthly meeting. Monitored by administrator.

NFPA 101 STANDARD Electrical Systems - Essential Electric Syste:This is a less serious (but not lowest level) deficiency but was found to be widespread throughout the facility and/or has the potential to affect a large portion or all the residents.  This deficiency is one that results in minimal discomfort to the resident or has the potential (not yet realized) to negatively affect the resident's ability to achieve his/her highest functional status.
Electrical Systems - Essential Electric System Maintenance and Testing
The generator or other alternate power source and associated equipment is capable of supplying service within 10 seconds. If the 10-second criterion is not met during the monthly test, a process shall be provided to annually confirm this capability for the life safety and critical branches. Maintenance and testing of the generator and transfer switches are performed in accordance with NFPA 110.
Generator sets are inspected weekly, exercised under load 30 minutes 12 times a year in 20-40 day intervals, and exercised once every 36 months for 4 continuous hours. Scheduled test under load conditions include a complete simulated cold start and automatic or manual transfer of all EES loads, and are conducted by competent personnel. Maintenance and testing of stored energy power sources (Type 3 EES) are in accordance with NFPA 111. Main and feeder circuit breakers are inspected annually, and a program for periodically exercising the components is established according to manufacturer requirements. Written records of maintenance and testing are maintained and readily available. EES electrical panels and circuits are marked, readily identifiable, and separate from normal power circuits. Minimizing the possibility of damage of the emergency power source is a design consideration for new installations.
6.4.4, 6.5.4, 6.6.4 (NFPA 99), NFPA 110, NFPA 111, 700.10 (NFPA 70)
Observations:

Based on document review and interview, the facility failed to maintain the emergency generator, affecting the entire facility.

Findings include:

Document review on January 28, 2026, at 11:30 a.m., revealed the facility failed to provide documentation for the following tests:

A. Monthly battery conductance test (sealed battery);
B. Annual 90-minute load bank;
C. Three-year, four-hour load test.

Interview with the maintenance supervisor on January 28, 2026, at 11:30 a.m., confirmed the documentation was unavailable at the time of the survey.





 Plan of Correction - To be completed: 02/24/2026

1. Documentation has been provided for the following tests on the emergency generator:
a. Monthly battery conductance test (sealed battery)
b. Annual 90 minute load bank
c. Three year, 4 hour load test.
2. Documentation will be maintained in a specific order in the documentation book for review of life safety individuals.
3. Quality Assurance will receive a monthly report of outside vendor tests conducted and monitored by the Maintenance Director.
