Based on an Onsite Revisit to a Recertification Survey completed on February 12, 2025, it was determined that Warren General Hospital was in substantial compliance with the requirements of the Life Safety Code for an existing critical access health care occupancy. Compliance with the National Fire Protection Association's Life Safety Code is required by 42 CFR 485.623(d).

This is a three-story, Type II (222), fire resistive building, with a penthouse and partial basement, that is partially sprinklered.

Compliance with the National Fire Protection Association's Life Safety Code is required by 42 CFR 485.623(d).

This is a one-story, Type II (111), protected, non-combustible building, that is fully sprinklered.

Based on an Onsite Revisit to a Recertification Survey completed on February 12, 2025, it was determined that Warren General Hospital was in substantial compliance with the requirements of the Life Safety Code for an existing acute care health care occupancy. Compliance with the National Fire Protection Association's Life Safety Code is required by 42 CFR 482.41(b).

This is a two-story, Type III (200), unprotected, ordinary building, that is not sprinklered.
Findings include:

Observation on February 12, 2025, at 1:25 p.m., revealed the facility lacked ground fault circuit interrupters (GFCI) at electrical outlets located within six feet of sinks throughout the building.

Interview with the facilities manager on February 12, 2025, at 1:25 p.m., confirmed the electrical outlets were not GFCI protected at the time of the survey.
