Initial comments:

Based on a Complaint Investigation, completed on October 1, 2024, at Milton Rehabilitation and Nursing Center, it was determined that there were no federal deficiencies identified under the requirements of 42 CFR Part 483, Subpart B, Requirements for Long Term Care; however, the facility was not in compliance with 28 PA Code, Chapter 51, of Pennsylvania Long Term Care Licensure Regulations.



 Plan of Correction:


51.4 (c) LICENSURE CHANGE IN OWNERSHIP & MANAGEMENT:State only Deficiency.
51.4. Change in ownership; change in management.

(c) A health care facility shall notify the Department in writing within 30 days after a change in management of a health care facility. A change in management occurs when the person responsible for the day to day operation of the health care facility changes.
Observations:

Based on staff interview, it was determined that the facility failed to notify the Department within 30 days of a change in management.

Findings include:

Interview with the Director of Nursing on October 1, 2024, at 11:15 AM revealed that the facility's management company changed on August 15, 2024. The Director of Nursing stated that the previous management company discontinued the facility staff's access to the bill payment system used, and the staff from the new management company would need to obtain evidence of invoices paid to the facility's contracted transportation providers before August 15, 2024.

Interview with the Director of Nursing on October 1, 2024, at 2:15 PM confirmed that the new management company is responsible for accounts payable and the day-to-day operations of the facility. She confirmed that there was no notice to the Department of Health regarding the change in management companies.


 Plan of Correction - To be completed: 10/18/2024

Facility will work on obtaining corporate level management changes and notify the state in writing.

No building wide action required.

RDO will educate administrator on notifying state of corporate level management changes.

Audit of corporate level management changes will be reviewed in QAPI October 2024.
