This report is for new equipment, Breast Biopsy Surgical Device, located at the Perelman Building, 3400 Civic Blvd, Philadelphia, Pa. 19104, in 3 East Surgicenter and the operating rooms (OR) four and five at the Pavilion Building, One Convention Avenue, Philadelphia, Pa. 19104, beginning on June 6, 2024; High Level Disinfecting Units (Model 2), located on the first floor of the Gates Building , Exam Rooms 8W and 10W, beginning on June 6, 2024; Ultrasound Unit (5500), located in the Pavilion Building, Level 7 Heart and Vascular Intensive Care Unit, One Convention Avenue, Philadelphia, Pa. 19104, beginning on June 6, 2024. The Hospital Of The University Of Pennsylvania attested they were in full compliance with the requirements of the Pennsylvania Department of Health's Rules and Regulations for Hospitals, 28 PA Code, Part IV, Subparts A and B, November 1987, as amended June 1998.
