Based on a Relicensure Survey completed on January 24, 2019, it was determined that American Access Care of SP ASC, LLC, was not in compliance with the following requirements of the Life Safety Code for an existing Ambulatory health care occupancy.

This ambulatory surgical facility is co-located with ID# 23111501, which is an existing building, with plan approval dated 2012.

This is a one-story, Type III (200), unprotected ordinary structure, which is not sprinklered.




 Plan of Correction:


28 Pa. Code 569.2 STANDARD Portable Fire Extinguishers:State only Deficiency.
Portable Fire Extinguishers
Portable fire extinguishers are selected, installed, inspected, and maintained in accordance with NFPA 10, Standard for Portable Fire Extinguishers.
20.3.5.3, 21.3.5.3, 9.7.4.1, NFPA 10
Observations:

Based on document review and interview, it was determined the facility failed to ensure inspection of portable fire extinguishers were performed by certified personnel, affecting the ambulatory surgery facility.

Findings Include:

1. Documentation reviewed on January 24, 2019, at 9:30 am, revealed documentation was unavailable verifying entities providing servicing and recharging of portable fire extinguishers had attended required training, and were credentialed.

Exit Interview with the Administrator on January 24, 2019, at 11:00 am, confirmed verification of portable fire extinguisher training was not available at the time of inspection.




 Plan of Correction - To be completed: 02/05/2019

Plan of Action:

1. Obtain a Certificate of training and credentials verifying that all entities providing servicing and recharging of portable fire extinguishers have attended required training are credentialed. Completed January 25, 2019.
2. Place the required documentation with the Fire Extinguisher reports in the Fire Safety & HVAC Binder kept in the Facility Administrators office. Completed January 25, 2019
