Initial comments:

This report is the result of a special monitoring survey for closure of the facility on February 19, 2025, at Pine Grove Ambulatory Surgery Center, Llc. It was determined the facility was not in compliance with the applicable requirements of the Pennsylvania Department of Health's Rules and Regulations for Ambulatory Care Facilities, Annex A, Title 28, Part IV, Subparts A and F, Chapters 551-573, November 1999.

Plan of Correction:

51.3 (c) LICENSURE Notification:

51.3 Notification

(c) A health care facility shall provide similar notice at least 60 days prior to the effective date it
intends to cease providing an existing health care service or reduce it licensed bed complement.

Observations:

Based on a review of facility documents and employee interview (EMP), it was determined the facility failed to notify the Department in writing at least 60 days prior to the effective date the facility intended to cease surgical services at the facility.

Findings Include:

Review, at approximately 9:15 AM on February 19, 2025, of "Pine Grove Ambulatory Surgical Center, LLC," dated December 5, 2024, revealed, "... Please accept this letter as the 60-day notice of closure of the Pine Grove Ambulatory Surgery Center, LLC. ... Our clinical operations will cease immediately following the end of the business on December 19, 2025 ..."

EMP1 confirmed the above finding at the time of observation.
