Initial comments:

Based on an Abbreviated Survey in response to a complaint completed on March 28, 2024, at Caring Heights Community Care and Rehabilitation Center it was determined that there were no federal deficiencies identified under the requirements of 42 CFR Part 483, Subpart B, Requirements for Long Term Care Facilities; however, the facility was not in compliance with 28 PA Code, Commonwealth of Pennsylvania Long Term Care Licensure Regulations.




 Plan of Correction:


§ 211.12(f.1)(4) LICENSURE Nursing services. :State only Deficiency.
(4) Effective July 1, 2023, a minimum of 1 LPN per 25 residents during the day, 1 LPN per 30 residents during the evening, and 1 LPN per 40 residents overnight.
Observations:

Based on review of nursing time schedules, resident interviews and staff interview, it was determined that the facility failed to provide a minimum of one licensed practical nurse (LPN) per 40 residents on the night shift on one out of 21 days (3/13/24).

Findings include:

Review of facility census data indicated that on 3/13/24, the facility census was 111, which required 2.78 licensed practical nurses (LPN's) during the overnight shift.

During an interview on 3/27/24, at 9:35 a.m. Resident R1 stated: "No problems with care. I think on the weekends they are short on staff."

During an interview on 3/27/24, at 9:42 a.m. Resident R2 stated: "Staff do ok. I do not think they have enough help here."

During an interview on 3/27/24, at 12:29 p.m. Resident R3 stated: "Staff tell me they are working short handed. They are doing right by me."

Review of nurse deployment (records detailing the nurses and nurse aides working each shift) dated 3/13/24, indicated that on the 3/13/24 overnight shift there was 3 LPN's working, and one of the assigned LPN worked only half an hour. The remaining overnight shift only had two LPN's.

During an interview on 3/28/24, at 10:35 a.m. the Staff scheduler Employee E1 confirmed that the facility failed to provide a minimum of one licensed practical nurse (LPN) per 40 residents during the overnight shift on 3/13/24.


 Plan of Correction - To be completed: 04/17/2024

Caring Heights Community Care and Rehabilitation Center provides this plan of correction without admitting or denying the validity or existence of the alleged deficiencies. The plan of correction is prepared and executed solely because it is required by federal and state law.

Nursing Home Administrator will re-educate Director of Nursing and Scheduler on staffing ratios regulation effective July 1, 2023.

Nursing Home Administrator/designee will audit staffing sheets weekly for four weeks to identify LPN ratio is met. Moving forward Nursing Home Administrator/designee will monitor the daily/weekly staffing sheets.

Nursing Home Administrator/designee is reviewing all current open positions on employment platform. Findings will be reported to QAPI.
