CARING HEIGHTS COMMUNITY CARE AND REHABILITATION CENTER - Inspection Results	Scope of Citation Number of Residents Affected By Deficient Practice

Initial comments:

Based on a revisit survey completed on July 2, 2026, it was determined that Caring Heights Community Care and Rehab Center corrected the deficiencies cited during the survey of June 2, 2026, under the requirements of 42 CFR Part 483, Subpart B Requirements for Long Term Care Facilities, however, has three continuing deficiencies under the requirements of the 28 Pa. Code, Commonwealth of Pennsylvania Long Term Care Licensure Regulations.

Plan of Correction:

§ 211.12(f.1)(3) LICENSURE Nursing services. :

(3) Effective July 1, 2024, a minimum of 1 nurse aide per 10 residents during the day, 1 nurse aide per 11 residents during the evening, and 1 nurse aide per 15 residents overnight.

Observations:

Based on a review of the facility's staffing worksheet and staff interview, it was determined that the facility failed to provide one Nurse Assistant (NA) per 10 residents on the daylight shift for three of six days (6/27/26, 6/28/26 and 6/30/26), one NA per 11 residents on the evening shift on six of six days (6/26/26 through 7/1/26) and one NA per 15 residents on the night shift for six of six days (6/26/26 through 7/1/26) as required.

Findings include:

A review of the facility's staffing worksheet for the time period of 6/26/26, through 7/1/26, revealed the following:

During an interview on 7/2/26 at 2:30 p.m., the Nursing Home Administrator confirmed that the facility failed to provide NA's on the above days as required.

Plan of Correction - To be completed: 07/29/2026

1. Director of Nursing, Assistant Director of Nursing, and staffing coordinator will be re-educated on the Pennsylvania staffing ratio regulation by the NHA/designee and specifically for nursing assistants.
2. Efforts to recruit are to continue to post employment ads on Indeed with occasional sign on bonus, interviews immediately, bonuses offered to pick up open shifts, and to use contracted staff for open shifts or call offs.
3. Efforts to maintain staff, include continuing to develop and post a monthly event calendar for staff, monthly IMPACT awards to recognize a staff member that goes above and beyond, referral bonuses for current staff that refer new nurses or care staff that are hired and bonuses for nursing staff that pick up open shifts.
4. NHA/designee will audit nursing schedules weekly for 4 weeks and then monthly for 2 months to ensure compliance.
5. Results of the audits will be reviewed by the QAPI committee x 4 months and then as directed by the committee

§ 211.12(f.1)(4) LICENSURE Nursing services. :

(4) Effective July 1, 2023, a minimum of 1 LPN per 25 residents during the day, 1 LPN per 30 residents during the evening, and 1 LPN per 40 residents overnight.

Observations:

Based on review of nursing time schedules and staff interview, it was determined that the facility administrative staff failed to provide a minimum of one licensed practical nurse (LPN) per 30 residents on the evening shift on two of six days (6/27/26 and 6/30/26) and one LPN per 40 residents on the night shift on four of six days (6/26/26, 6/27/26, 6/30/26 and 7/1/26) as required.

Findings include:

Review of facility census data and nursing time schedules from 6/26/26 through 7/1/26 revealed the following LPN staffing shortage:

§ 211.12(i)(2) LICENSURE Nursing services.:

(2) Effective July 1, 2024, the total number of hours of general nursing care provided in each 24-hour period shall, when totaled for the entire facility, be a minimum of 3.2 hours of direct resident care for each resident.

Observations:

Based on a review of nursing time schedules and staff interview, it was determined that the facility failed to provide a minimum of 3.20 PPD (per patient daily) hours of direct care for each resident on six of six days (6/26/26 through 7/1/26).

Findings include:

Review of staffing documents and nursing staff schedules from 6/26/26 through 7/1/26, indicated that the State required PPD minimum hours of 3.20 was not met on the following days:

6/26/26= 2.85 PPD.
6/27/26= 2.75 PPD.
6/28/26= 2.62 PPD.
6/29/26= 2.88 PPD.
6/30/26= 2.73 PPD.
7/1/26= 2.95 PPD.

During an interview on 7/2/26 at 2:30 p.m. the Nursing Home Administrator confirmed that the facility failed to provide a minimum of 3.20 PPD hours of direct care on the above dates as required.

Plan of Correction - To be completed: 07/29/2026

1. DON/designee will re-educate the staffing coordinator on the Pennsylvania staffing ppd regulation of 3.2 hours of general nursing care to each resident in a 24-hour period.
2. Efforts to recruit are to continue to post employment ads on Indeed with occasional sign on bonus, interviews immediately and to use contracted staff for open shifts or call offs.
3. Efforts to maintain staff, include continuing to develop and post a monthly event calendar for staff, monthly IMPACT awards to recognize a staff member that goes above and beyond, referral bonuses for current staff that refer new nurses or care staff that are hired.
