Initial comments:


Based on a Follow-up Survey completed on March 28, 2024, it was determined that Lakeview Healthcare and Rehab was not in compliance with the following requirements under the 28 PA Code, Commonwealth of Pennsylvania Long Term Care Licensure Regulations.



 Plan of Correction:


211.12(f.1)(5) LICENSURE Nursing services. :State only Deficiency.
(5) Effective July 1, 2023, a minimum of 1 RN per 250 residents during all shifts.
Observations:


Based on review of the facility staffing documents and staff interview, it was determined that the facility failed to ensure a minimum of one RN per 250 residents worked during all shifts for two of seven days reviewed (3/03/24 and 3/05/24).

Findings include:

Review of facility staffing ratio information from 3/03/24 through 3/09/24, revealed the following RN staffing shortages for the overnight shifts where the RN ratios were not met:

3/03/24 census of 25 residents 0.25 RNs worked and 1.0 was required
3/05/24census of 25 residents0.50 RNs worked and 1.0 was required

Review of an email correspondence interview dated 3/27/24, at 3:19 p.m. revealed the Nursing Home Administrator confirmed the RN ratios were not met for the above dates and shift.




 Plan of Correction - To be completed: 04/12/2024

5540 Effective July 1, 2023, a minimum of 1 RN per 250 resident during all shifts.
In accordance with Pennsylvania Code 211.12, subsection (f.2)(3), "a facility with a census of 59 or under may substitute an LPN for an RN on the overnight shift only if an RN is on call and located within a 30-minute drive of the facility". The facility Nursing Home Administrator will educate the Director of Nursing and supervisors on staffing ratios utilizing both RN and LPN/LPN Charge adhering to Pennsylvania Code 211.12. The facility has executed three nursing agency contracts, hired two RN's and continued to offer shift bonuses to ensure appropriate ratios are being met. The facility Nursing Home Administrator or designee will review the direct care schedule daily to ensure that appropriate staffing ratios are being met. The Nursing Home Administrator or designee will conduct a random audit of direct care deployment sheets for RN ratios being met daily x5, weekly x2 to ensure staffing ratios are met with results to the facility Quality Assessment and Assurance Committee.
