Based on a Relicensure Survey completed on September 27, 2023, it was determined that Allegheny Regional Endoscopy Center was not in compliance with the following requirements of the Life Safety Code for an existing Ambulatory health care occupancy. Compliance with the National Fire Protection Association's Life Safety Code is required by 42 CFR 416.44(b).

This is a one-story, Type III (200), unprotected ordinary building, with a basement, that is fully sprinklered.






 Plan of Correction:


28 Pa. Code § 569.2 STANDARD Electrical Systems - Other:State only Deficiency.

List in the REMARKS section, any NFPA 99 Chapter 6 Electrical Systems requirements that are not addressed by the provided S-Tags, but are deficient.
Observations:
Name: MAIN BUILDING - Component: 01 - Tag: 0911

Based on observation and interview, it was determined the facility failed to maintain electrical wiring in one instance, affecting one of two floors. Installation shall be in accordance with NFPA 70, National Electric Code. 6.3.2.1, NFPA 99.

Findings include:

1. Observation on September 27, 2023, at 10:17 a.m., revealed an unterminated MC electric cable in the basement hot water heater room, behind the hot water heaters.

Interview with the Facility Director and the Maintenance Director on September 27, 2023, at 12:30 p.m., confirmed the above listed electrical wiring deficiency.





 Plan of Correction - To be completed: 10/09/2023

The Plan of Correction to address the listed deficiency of an unterminated MC electrical in the basement hot water room is to remove the circuit and associated cabling in entirety back to the circuit breaker box thus completely eliminating the cable in its entirety. The cable removal was completed on 09-29-2023.

Going forward, the administrator will be notified by the maintenance person when electrical service points are removed so that an audit can be conducted by the administrator to ensure all cabling is properly terminated or removed.
