Based on an Occupancy Survey completed on November 20, 2023, it was determined there were no deficiencies identified as related to the requirements of the Life Safety Code for an existing Ambulatory health care occupancy.

This Phase 1 Occupancy Survey was conducted for alterations to the fifth floor Pharmacy to comply with USP 795, USP 797 and USP 800. Create a break room for the pharmacy on the first floor (ASC locatd on the fourth floor).

This a five story, Type II (222), fire resistive construction, which is fully sprinklered.

Approved as a Class B Ambulatory Surgical Facility.
