Based on a Relicensure Survey completed on June 29, 2023, it was determined that Hazleton Endoscopy Center was not in compliance with the following requirements of the Life Safety Code for an existing Ambulatory health care occupancy.

This is a two story, Type V (000), unprotected, wood-frame structure which is fully sprinklered.




 Plan of Correction:


28 Pa. Code § 569.2 STANDARD Fire Alarm System - Testing and Maintenance:State only Deficiency.
Fire Alarm Systems - Testing and Maintenance
A fire alarm system is tested and maintained in accordance with an approved program complying with the requirements of NFPA 70, National Electric Code, and NFPA 72, National Fire Alarm and Signaling Code. Records of system acceptance, maintenance and testing are readily available.

Observations:
Name: MAIN BUILDING 01 - Component: 01 - Tag: 0345

Based on documentation review and interview, it was determined the facility failed to maintain fire alarm systems, affecting the entire facility.

Findings include:

1. Review of documentation on June 29, 2023, at 8:45 a.m., revealed the facility lacked records to support the required semi-annual inspection of the fire alarm system.

Exit interview with the facility representative #1 on June 29, 2023, at 10:20 a.m., confirmed the lack of documentation.






 Plan of Correction - To be completed: 08/09/2023

Vector Security has been contacted regarding the semi-annual inspection of the fire alarm system.

The inspection is scheduled for July 28th, 2023.

The completion date is 8/09/2023 and will be reviewed at least semi-annually by the Center Director to ensure timely completion.

28 Pa. Code § 569.2 STANDARD Gas and Vacuum Piped Systems - Maintenance:State only Deficiency.

Observations:
Name: MAIN BUILDING 01 - Component: 01 - Tag: 0907

Based on documentation review and interview, it was determined the facility failed to maintain piped in medical gas components, affecting one of two floors.

Findings include:

1. Documentation review on June 29, 2023, at 9:05 a.m., revealed the facility lacked a required annual Medical Gas System report.

Exit interview with the facility representative #1 on June 29, 2023, at 10:20 a.m., confirmed the lack of documentation.





 Plan of Correction - To be completed: 08/09/2023

Sherman Engineering has been contacted.

The annual Medical Gas System inspection is scheduled for July 12th, 2023.

This will be completed by 08/09/2023 and will be reviewed at least annually by the Center Director to ensure timely completion.
