Initial comments:

Based on a Medicare/Medicaid Recertification, State Licensure, and Civil Rights Compliance survey completed on January 8, 2026, it was determined that Good Shepherd Home-Bethlehem was not in compliance with the following requirements of 42 CFR Part 483, Subpart B, Requirements for Long Term Care and the 28 Pa. Code, Commonwealth of Pennsylvania Long Term Care Licensure Regulationsthey relate to the Health portion of the survey.


















 Plan of Correction:


483.60(i)(4) REQUIREMENT Dispose Garbage and Refuse Properly:Least serious deficiency but was found to be widespread throughout the facility and/or has the potential to affect a large portion or all the residents. This deficiency has the potential for causing no more than a minor negative impact on the resident.
§483.60(i)(4)- Dispose of garbage and refuse properly.
Observations:

Based on observation, it was determined that the facility failed to dispose of trash and refuse properly.

Findings include:

Observation of the dumpster area on January 6, 2025, at 10:53 a.m., revealed the dumpster was full of trash bags and the top lid was broken exposing trash.

 Plan of Correction - To be completed: 02/24/2026

1. It was not identified that any resident was impacted.
2. The dumpster identified was emptied and the top lid was replaced with a new lid.
3. The NHA will complete re-education with housekeeping and maintenance on regulation F 814 requirements.
4. The NHA/ designee will audit the dumpster area twice weekly times two months and the once weekly times one month to ensure trash and refuse is disposed of properly. Any concerns identified will be addressed at the time of the finding. The NHA will report a summary of the audits at the monthly quality meeting.
