Based on a Revisit to a Medicare/Medicaid Recertification Survey completed on March 26, 2025, it was determined that Yardley Rehabilitation and Healthcare Center - Main Building was not in substantial compliance with the following requirements of the Life Safety Code for an existing Nursing health care occupancy. Compliance with the National Fire Protection Association's Life Safety Code is required by 42 CFR 483.90(a).

This is a one-story, Type V (111), protected wood frame building, that is fully sprinklered.








 Plan of Correction:


NFPA 101 STANDARD Subdivision of Building Spaces - Smoke Compar:Least serious deficiency but was found to be widespread throughout the facility and/or has the potential to affect a large portion or all the residents. This deficiency has the potential for causing no more than a minor negative impact on the resident.

Smoke barriers shall be provided to form at least two smoke compartments on every sleeping floor with a 30 or more patient bed capacity. Size of compartments cannot exceed 22,500 square feet or a 200-foot travel distance from any point in the compartment to a door in the smoke barrier.

Observations:
Name: MAIN BUILDING 01 - Component: 01 - Tag: 0371

Based on observation, document review, and interview, it was determined the facility failed to provide smoke compartments not greater than 22,500 square feet, with a travel distance not to exceed 200 feet, affecting one of six smoke compartments within the component.

Findings include:

1.Observation and document review on March 26, 2025, at 10:00 a.m., revealed the total area for Smoke Compartment One was in excess of 22,500 square feet. Smoke Compartment One included the mechanical room, kitchen, main laundry room to the old Heritage dining room, the main entrance and the administration area to the main dining room.

Exit Interview with the Administrator and Maintenance Director on March 26, 2025, at 12:00 p.m., confirmed the smoke compartment exceeded the maximum square footage allowance.

Observations during an onsite Revisit conducted on May 7, 2025, between 11:00 a.m. and 1:00 p.m., determined the following:

Item 1 - Not Completed. The total area for Smoke Compartment One was in excess of 22,500 square feet. Smoke Compartment One included the mechanical room, kitchen, main laundry room to the old Heritage dining room, the main entrance and the administration area to the main dining room. FSES was not provided at time of Revisit as indicated on Facility Plan of Correction.

Interview at the exit conference with the Administrator and Maintenance Director on January 12, 2023, at 2:30 p.m., confirmed the above item was not completed.

The facility will perform an FSES inspection and complete the designated worksheets in conjunction with a licensed architect/engineer.
