Based on a Relicensure Survey completed on August 6, 2024, it was determined The Surgery and Laser Center was not in compliance with the following requirements of the Life Safety Code for an existing ambulatory health care occupancy. Compliance with the National Fire Protection Association's Life Safety Code is required by 42 CFR 416.44(b).

This is a one-story, Type V (000) , unprotected, wood frame building, with a basement, that is not sprinklered.





 Plan of Correction:


28 Pa. Code § 569.2 STANDARD Gas and Vacuum Piped Systems - Inspection:State only Deficiency.
Gas and Vacuum Piped Systems - Inspection and Testing Operations
The gas and vacuum systems are inspected and tested as part of a maintenance program and include the required elements. Records of the inspections and testing are maintained as required.
5.1.14.2.3, B.5.2, 5.2.13, 5.3.13, 5.3.13.4 (NFPA 99)
Observations:
Name: MAIN BUILDING 01 - Component: 01 - Tag: 0908

Based on document review and interview, the facility failed to maintain gas and vacuum piped systems for one of one component.

Findings include:

Document review on August 6, 2024, at 8:10 a.m., revealed the manifold room pigtails had an unknown installation date and were suspected to be in service longer than five years.

Interview with the facility director on August 6, 2024, at 8:10 a.m., confirmed the deficiency and stated that the facility had replacement parts.




 Plan of Correction - To be completed: 08/13/2024

The copper tails were purchased from Med Gas. The tails were installed and tested August 12, 2024

The five year copper tail change has been added to the critical date calendar in the computer system by the Center Director. This is monitored monthly by the Center director.

The copper tails were each tagged with installation and change dates by the Center Director.

The corrective action was completed August 12, 2024

Electrical Systems - Essential Electric System Maintenance and Testing
The generator or other alternate power source and associated equipment is capable of supplying service within 10 seconds. If the 10-second criterion is not met during the monthly test, a process shall be provided to annually confirm this capability for the life safety and critical branches. Maintenance and testing of the generator and transfer switches are performed in accordance with NFPA 110.
Generator sets are inspected weekly, exercised under load 30 minutes 12 times a year in 20-40 day intervals, and exercised once every 36 months for four continuous hours. Scheduled test under load conditions include a complete simulated cold start and automatic or manual transfer of all EES loads, and are conducted by competent personnel. Maintenance and testing of stored energy power sources (Type 3 EES) are in accordance with NFPA 111. Main and feeder circuit breakers are inspected annually, and a program for periodically exercising the components is established according to manufacturer requirements. Written records of maintenance and testing are maintained and readily available. EES electrical panels and circuits are marked and readily identifiable. Minimizing the possibility of damage of the emergency power source is a design consideration for new installations.

Observations:
Name: MAIN BUILDING 01 - Component: 01 - Tag: 0918

Based on observation, document review, and interview, the facility failed to maintain essential electrical system requirements, affecting the entire facility.

Findings include:

Observation and document review on August 6, 2024, between 8:33 a.m. and 8:40 a.m., revealed the following generator deficiencies:
A. (8:33 a.m.) Last documented three-year, four-hour load test was completed on July 30, 2021;
B. (8:40 a.m.) Basement transfer switch had a red alert light illuminated.

Interview with the facility director on August 6, 2024, at 8:40 a.m., confirmed the deficiencies at the time of the survey. The director stated that Palco was scheduled to do testing on August 12, 2024.




 Plan of Correction - To be completed: 08/13/2024

The 4 hour load bank test is scheduled every 4 years. The test was schedudled in July the soonest we could be scheduled was August 12, 2024. In the future the test will be scheduled every 4 years for an early August appointment giving Palcos enough time to schedule out the test with their staffing.

The critical date was changed in the computer system to read schedule in June every 4 years by the Center Director.

Palcos completed the 4 hour load bank test August 12,2024
