Based on observation and interview, it was determined the facility failed to maintain electrical panels, to be accessible, in one of two smoke zones within the component.

Findings include:

1. Observation on August 8, 2023, at 11:59 AM, revealed carts and trash cans were stored against the Electrical Room door, across from Stairtower B, on the 2nd floor.

Plan of Correction - To be completed: 08/25/2023

0911 – Failure to maintain electrical panels, to be accessible, in one of the two smoke zones within the component due to moveable carts being placed in front of the electrical closet doors, across from Stairtower B, on the 2nd floor.

The plan of correction to prevent this from occurring in the future is:
1. Place signage on the electrical closet doors stating, "Do Not Block Door to this Electrical Closet".
To be completed by 8/25/2023.
2. Training of staff on the need to keep from moving any carts or storage in front of these electrical closet doors, as well as in the means of egress to the stairtower.
To be completed by 8/25/2023.
3. During normal building tours, staff will survey this area to ensure the doors are not blocked so the electrical closet panels are accessible.
This will be performed on a weekly basis beginning 8/28/23.
