Based on a Medicare/Medicaid Recertification Survey completed on February 11, 2025, it was determined that Chambers Pointe Health Care Center was not in compliance with the following requirements of the Life Safety Code for an existing health care occupancy. Compliance with the National Fire Protection Association's Life Safety Code is required by 42 CFR 483.90(a).

This is a two-story, Type II (111), protected noncombustible structure, without a basement, which is fully sprinklered.



 Plan of Correction:


NFPA 101 STANDARD Building Construction Type and Height:This is a less serious (but not lowest level) deficiency and affects more than a limited number of residents, staff, or occurrences. This deficiency is one that results in minimal discomfort to the resident or has the potential (not yet realized) to negatively affect the resident's ability to achieve his/her highest functional status. This deficiency was not found to be throughout this facility.

Observations:
Name: MAIN BUILDING - Component: 01 - Tag: 0161

Based on observation and interview, it was determined the facility failed to maintain the rated horizontal fire doors, to self-close and positively latch, affecting one of six smoke compartments within the component.
Findings include:
1. Observation on February 11, 2025, at 11:30 AM, revealed the main floor Janitorial Closet, by Resident Room 536, attic horizontal fire-rated access door failed to self-close and latch in frame.
Interview at the time of the exit conference with the Administrator and Director of Maintenance on February 11, 2025, at 1:30 PM, confirmed the horizontal fire-rated access door would not self-close and positively latch.



 Plan of Correction - To be completed: 03/17/2025

1. The attic horizontal fire-rated access door will have springs properly installed to make the door self-close and latch in the frame.
2. The access door will be added to quarterly inspection PMs and the annual fire door inspection PM to check for proper operation and documentation. Any issues will be reported to the Manager of Safety/Security for proper repairs to be made. Any issues will also be reported to the Nursing Home Administrator to assure proper follow-up.
