Initial comments:


Based on a revisit survey completed on January 30, 2024, it was determined that Eldercrest Healthcare and Rehabilitation failed to correct the deficiency identified during the survey of December 19, 2023, as related to the requirements of the 28 PA Code, Commonwealth of Pennsylvania Long Term Care Licensure Regulations.




 Plan of Correction:


§ 211.12(f.1)(4) LICENSURE Nursing services. :State only Deficiency.
(4) Effective July 1, 2023, a minimum of 1 LPN per 25 residents during the day, 1 LPN per 30 residents during the evening, and 1 LPN per 40 residents overnight.
Observations:


Based on review of nursing time schedules and staff interview, it was determined that the facility administrative staff failed to provide a minimum of one licensed practical nurse (LPN) per 25 residents during the day shift on three of seven days (1/25/24, 1/27/24 and 1/28/24), one LPN for 30 residents on the evening shift on seven of seven days (1/23/24 through 1/29/24).

Findings include:

Review of the facility census data and nursing time schedules from 1/23/24 through 1/29/24 revealed the following LPN staffing shortages:

Day shift:

1/28/24census 38actual hours 8.0required hours 12.16.

Evening shift:

1/26/24census 40actual hours 8.0required hours 10.67.

1/29/24census 38actual hours 8.0required hours 10.13.

During an interview on 1/30/24, at 9:21 a.m. the Nursing Home Administrator confirmed that the facility to provide licensed practical nurses on the above dates as required.



 Plan of Correction - To be completed: 03/12/2024

1) The facility will continue to take measures to adequately staff and meet the LPN ratios of direct nursing care per resident day.
2) Education will be completed on PA SNF LPN staffing ratios with DON, ADON and HR/Scheduler.
3) The NHA or designee will conduct daily labor meetings and audit daily schedules to ensure minimum number of LPN nursing care staff per resident are scheduled to meet the needs of the residents.
4) To help increase staffing, facility has Agency contracts, recruitment efforts with social media, referral and sign on bonuses.
5) Audits will be conducted daily for 14 days and then weekly for 3 weeks.
6) The results will be reviewed at Quality Assurance and Process Improvement meetings until substantial compliance has been met.
