Based on an Occupancy Survey conducted on April 21, 2025, and completed on April 24, 2025, it was determined there were no deficiencies identified as related to the requirements of the Life Safety Code for an existing Nursing health care occupancy.

This Phase 1 Occupancy Survey was conducted for Alterations to renovate the existing skilled nursing facility, including nurse call replacement throughout the facility. Phase 2 of a two-phased project.

Areas Inspected: Dining room, physical therapy room, nurse station, medication room, staff break room, elevator shaft

Refer to Drawing Numbers H-24-0568, Revision H-25-0004, Sprinklers H-25-0326 .

This is a one story, Type V (111), protected wood frame building, which is fully sprinklered.
