Initial comments:

Based on an Abbreviated survey completed on February 16, 2024, at Harborview Rehabilitation Care Center- Doylestown, it was determined that there were no federal deficiencies identified under the requirements of 42 CFR Part 483, Subpart B Requirements for Long Term Care; however, the facility was not in compliance with the 28 Pa. Code, Commonwealth of Pennsylvania Long Term Care Licensure Regulations.




 Plan of Correction:


§ 211.12(i)(1) LICENSURE Nursing services.:State only Deficiency.
(1) Effective July 1, 2023, the total number of hours of general nursing care provided in each 24-hour period shall, when totaled for the entire facility, be a minimum of 2.87 hours of direct resident care for each resident.

Observations:

Based on review of nursing hours as worked from February 6 , through February 15 , 2024, and staff interview, it was determined that the facility failed to ensure that 2.87 hours of direct care per resident was provided for each resident for two of 10 days reviewed.

Findings include:

Review of nursing hours as worked from February 6, through February 15, 2024, revealed that on two days the regulatory requirments of 2.87 hours of direct care per resident was not met.

In an interviw on February 16, 2024, at 11:30 a.m., the Nursing Home Administator confirmed that on two days, the hours were not met.








 Plan of Correction - To be completed: 02/27/2024

Facility will review schedules daily to assure that required 2.87 hours of care are met.
- Facility scheduler, DON and Administrator will meet daily to review the current and next 3-day schedules to assure compliance.

- Any staffing issues will be reported to QAPI
