Initial comments:

Based on an Abbreviated Survey in response to two Complaint Investigations completed on April 1, 2025, at Centre Care Rehabilitation and Wellness Services, it was determined that there were no federal deficiencies identified under the requirements of 42 CFR Part 483, Subpart B, Requirements for Long Term Care; however, the facility was not in compliance with 28 PA Code, Commonwealth of Pennsylvania Long Term Care Licensure Regulations as they relate to the Health portion of the survey process.


 Plan of Correction:


§ 211.12(f.1)(3) LICENSURE Nursing services. :State only Deficiency.
(3) Effective July 1, 2024, a minimum of 1 nurse aide per 10 residents during the day, 1 nurse aide per 11 residents during the evening, and 1 nurse aide per 15 residents overnight.

Observations:

Based on a review of nursing staffing hours and staff interview, it was determined that the facility failed to ensure a minimum of one nurse aide (NA) per 10 residents during the day shift for 18 of the 21 days reviewed, failed to ensure a minimum of one NA per 11 residents during the evening shift for 21 of the 21 days reviewed, and failed to ensure a minimum of one NA per 15 residents during the overnight shift for 18 of the 21 days reviewed.

Findings include:

Review of nursing staff care hours provided by the facility for February 23 to March 1, 2025, March 18 to March 24, 2025, and March 25 to March 31, 2025, revealed the following NAs scheduled for the resident census:

Interview with the Nursing Home Administrator and Director of Nursing on April 1, 2025, at 3:15 PM confirmed that the facility did not meet regulatory NA-to-resident ratios as evidence above.

Preparation and/or execution of this plan of correction does not constitute admission or agreement by the provider of the truth of the facts alleged or conclusions set forth in the statement of deficiencies. The plan of correction is prepared and/or executed solely because it is required by the provisions of federal and state law.

The administrator, director of nursing, scheduler, and human resource director will be educated on the state requirement for nursing hours, including the certified nurse aide-to-resident ratios, by the quality clinical consultant/designee. Staffing meetings will be held 5 days a week to review the certified nurse aide ratio from the previous day and the projected certified nurse aide ratio for the current day, as well as the upcoming week, to ensure appropriate staffing levels by the nursing home administrator/designee. If projected staffing ratios do not meet the minimum, then the facility will reach out to current staff and local staffing agencies to enlist to meet the minimum requirement. The facility will continue to recruit staff through all platforms.

§ 211.12(i)(2) LICENSURE Nursing services.:State only Deficiency.
(2) Effective July 1, 2024, the total number of hours of general nursing care provided in each 24-hour period shall, when totaled for the entire facility, be a minimum of 3.2 hours of direct resident care for each resident.

Observations:

Based on review of nursing staffing hours and staff interview, it was determined that the facility failed to ensure the total number of nursing care hours provided in each 24-hour period was a minimum of 3.2 hours per patient day (PPD), effective July 1, 2024, for 21 of 21 days reviewed.

Findings include:

The administrator, director of nursing, scheduler, and human resource director will be educated on the state requirement for nursing hours, including the nurse-to-resident ratios, by the quality clinical consultant/designee.
Staffing meetings will be held 5 days a week to review HPPD from the previous day and the projected HPPD for the current day, as well as the upcoming week, to ensure appropriate staffing levels by the nursing home administrator/designee. If projected staffing levels are below the minimum of 3.20, then the facility will reach out to current staff and local staffing agencies to enlist to meet the minimum requirement. The facility will continue to recruit staff through all platforms.
