Overview
The Nursing Care Facility Performance Profile page provides a comparison of user selected facilities
by displaying information related to deficiencies identified via the
Commonwealth Inspection Process within the previous 30-month period.
For each Nursing Care Facility a raw count of deficiencies by
regulation category is displayed and a bar graph is shown depicting the
number of deficiencies within each level of severity (No Harm, Minimal Harm,
Actual Harm, Serious Harm) for the Facility, compared to the statewide
average number of deficiencies, and the average number of deficiencies for
facilities of similar size.
Number of Citations in Each Category
This section of the page displays
a raw numeric count of deficiencies found during the Commonwealth
inspection process within the previous 30 months. The deficiencies have
been grouped based upon Regulation Tag Groups as
follows:
| Resident Rights: Admission,
Transfer and Discharge |
Regulatory
requirements which require that residents have a right to be treated with dignity; should have the ability to make their own decisions and choices; have the right to speak with, write to and visit with whomever they desire, and have the right to access services inside and outside the facility. |
| Staff Treatment of Residents |
Regulatory requirements require that residents should be treated in a manner and in an environment that is free from abuse. |
| Quality of Life |
Regulatory
requirements pertaining to the care provided in an environment that is maintained to enhance each residents dignity and respect in full recognition of his/her individuality. |
| Resident Assessment |
Regulatory
requirements pertaining to a complete and accurate evaluation of the residents physical, mental and social abilities. |
| Quality of Care |
Regulatory
requirements pertaining to the care that a resident must receive to achieve and maintain the residents highest possible mental, physical and social well being. |
| Nursing and Physician Services |
Regulatory
requirements that pertain to ensuring that nursing and physician
services are provided to residents. These requirements include
ensuring that the facility has sufficient staff to meet the needs of
the resident, that Registered Nurses are utilized at least eight consecutive hours per day, and that the physician must supervise the medical care of each resident. |
| Dietary Services |
Regulatory
requirements that pertain to the provision of nutritional support to residents. These may include meal services, supplementation and consultation to ensure that residents' dietary needs are met. |
| Ancillary Services |
Regulatory
requirements that pertain to the services of physical therapy, occupational therapy, speech therapy, rehabilitative services, dental services, pharmacy services and any other service that the resident may need. |
| Physical Environment |
Regulatory
requirements involving building safety from fire, emergency power, physical plant, furniture, call systems, lighting, ventilation and pest control. |
| Administration |
Regulatory
requirements pertaining to the owner, board of directors, nursing home administrator (NHA) and director of nursing (DON) that make up the administration of the facility. They are responsible for managing the facility to ensure each residents needs are met. |

Level of Potential Harm to Resident Deficiency Comparison
The
bar graphs to the right of
the screen provide a deficiency comparison between the Facility, the Statewide
average, and average of Facilities of Similar Size. With each bar
on the graph from zero to three color-coded segments are displayed, to
represent the number of deficiencies found within each level of potential
harm: No Harm (Blue), Minimal Harm (Green), Actual Harm (Yellow),
and Serious Harm (Red). (Further information regarding level of potential

deficiencies within a level of severity the representative color will
not be displayed within the bar.
