Overview
The Nursing Care
Facility Information page displays a list of facilities that matched
the search criteria entered previously on the Nursing Care Facility
Locator page. The search criteria entered are summarized at the top of
the page. In addition, pertinent high level information about each
facility is provided, as well as links to each facilities Survey
Results and Additional Services . Facilities can be
compared, in terms of deficiencies found during the Commonwealths
Inspection process, by choosing either the Compare Selected
Facilities or Compare All Facilities
options.
Description of Data Displayed on
Nursing Care Facility Information
For each facility
displayed on the page, the following information will be provided:

Description |
Name/Address/Phone |
The facilitys legal name will appear in bold. If
the facility also has other common or community known names, these
will appear above the legal name within parenthesis and in
italics. |
Type of Ownership |
Indicates how the facility
reported to the department when completing ownership information,
such as profit, nonprofit, county, state owned or other. An example
of "other" ownership might be a facility that is owned by the
federal government. |
Licensure Status |
Represents the current status of the Facilitys
state license. Regular means the facility has a full
license. Provisional I, Provisional II, Provisional III,
Provisional IV designate lesser status due to deficiencies
cited during the inspection process. |
Last Inspection |
This is the date of the last
completed inspection for which the Survey Results can be found via
this web site. The survey results are placed on the web site
approximately 45 days after the conclusion of the onsite from the department. |
Size of Facility |
Facilities are categorized based on the number of
beds each contains. Small is 0 to 119,
Medium is 120 to 269, Large is 270 or
more beds. |
Number of Beds |
The number of currently licensed beds in the
facility. |
Payment Options |
The methods of payment accepted by the
facility. Payment mechanisms include Private Payment
, Medicare, Medicaid or any combination
of the three payment types. |
Nursing Hours Per Resident Per
Day |
The number of direct care staff
provided per resident each day. The state regulations
require that each facility provide 2.7 hours of direct care from the
nursing staff each day per resident. If the residents require more than the minimum number of
hours designated by the regulations, the facility is expected
to provide care based on those needs. That may be why some facilities have a
higher number of nursing hours than
others. |
Accessing Facility Survey
Results
Clicking the link labeled View Surveys
under the facility phone number will access each facilitys survey
results. The survey results describe any deficiencies found during the
inspection process over the previous 30-month period.
Accessing Facility Additional Services
Clicking
the link labeled View Additional Services will access
a list of extra services and information provided directly by each facility. Please note
that not all facilities have elected to provide this additional
information for this web site, you may wish to contact the
facility directly. The Commonwealth is not responsible for the accuracy of this
information.
Compare
Facilities
This web site offers the opportunity to compare facilities based
on deficiencies found during the Department of Health's inspection
process over the previous 30-month period, via the Performance Profile.
You may select specific facilities (one or more) to compare by clicking
the box to the left of the Facility name, so that a check mark is
displayed within the box, and choosing the Compare Selected
Facilities button. If you wish to choose all facilities listed on
this page you simply need to click on the Compare All Facilities
button. Both of these options will launch the Performance Profile
page. You may erase all selections by clicking the Clear
Selections button.
