Initial Comments:

This report is the result of a full Medicare Recertification survey conducted on April 29, 2014, at Fastrad Pennsylvania LLC. It was determined that the facility was not in compliance with the requirements of 42 CFR, Title 42, Part 486 - Conditions of Participation for a Portable X-ray Provider.







Plan of Correction:




486.102(a) STANDARD
PHYSICIAN SUPERVISION

Name - Component - 00
The performance of the roentgenologic procedures must be subject to the supervision of a physician who meets the requirements of 405.1412(b) and one of the following requirements must be met:

(1) The supervising physician owns the equipment and it is operated only by his employees, or

(2) The supervising physician certifies annually that he periodically checks the procedural manuals and observes the operators' performance, that he has verified that equipment and personnel meet applicable Federal, State, and local licensure and registration requirements and that safe operating procedures are used.



Observations:


Based on review of facility documents and staff interviews (EMP), it was determined that the supervising physician failed to review procedural manuals and observe operator performance annually.

Findings include:

Review of facility documents revealed that the supervising physician has not provided a "review of techniques and procedures with x-ray technologies and review of procedure on diagnostic services," since April 22, 2010.

Interview on April 29, 2014, at approximately 1:45 PM, with EMP1 confirmed there was no documented evidence that the supervising physician reviewed the procedural manuals and observed the operator's performance annually.




Plan of Correction:

Supervising Physician has reviewed and approved all policy and procedure manuals and observed operator performance. Fastrad PA Administrator will be responsible for monitoring performance of Supervising Physician and will maintain annual evidence of same. Administrator will insure Supervising Physician will review techniques and procedures with xr technologies and review of procedures of diagnostic services annually. These records will be available for future inspections.


486.110(b) STANDARD
RECORDS OF INSPECTION/SCOPE OF INSPECTION

The supplier must maintain records of current inspections which include the extent to which equipment and shielding are in compliance with the safety standards outlined in 486.108.



Observations:


Based on review of facility documents and staff interviews (EMP), it was determined that the facility failed to maintain records of inspections of protective thyroid collars.

Findings include:

Review of facility documents failed to address the testing of thyroid collars for integrity.

Interview on April 29, 2014 , at approximately 2:30 PM, with EMP1 confirmed there was no documented evidence that protective equipment was tested to ensure compliance with safety standards.







Plan of Correction:

Physicist has inspected all protective equipment including Thyroid Collars and documented same. Fastrad PA Administrator will monitor physiciast inspection of all protective devises and will maintain documented inspections of protective equipment.
