Based on a Revisit to a Medicare/Medicaid Recertification Survey completed on August 13, 2018, it was determined that Brookside Healthcare And Rehabilitation Center, Main Building was in compliance with the requirements of the Life Safety Code for an existing Nursing health care occupancy. Compliance with the National Fire Protection Association's Life Safety Code is required by 42 CFR 483.70(a).

This is a one-story, Type V (000), unprotected wood frame construction, with a partial basement, which is fully sprinklered.

Based on a Medicare/Medicaid Recertification Survey completed on August 13, 2018, at that Brookside Healthcare And Rehabilitation Center, Physical Therapy Addition, it was determined there were no deficiencies identified under the requirements of the Life Safety Code for an existing health care occupancy. Compliance with the National Fire Protection Association's Life Safety Code is required by 42 CFR 483.90(a).

This is a one-story building, Type III (111), protected non-combustible construction, which is fully sprinklered.
