Initial comments:

Based on an abbreviated complaint survey completed on March 20, 2019, at Ridgeview Healthcare and Rehabilitation Center, it was determined that there were no federal deficiencies identified under requirements of 42 CFR Part 483, Subpart B, Requirements for Long Term Care; however, the facility was not in compliance with 28 PA Code Commonwealth of Pennsylvania Long Term Care Licensure Regulations.



 Plan of Correction:


211.12(i) LICENSURE Nursing services.:State only Deficiency.
(i) A minimum number of general nursing care hours shall be provided for each 24-hour period. The total number of hours of general nursing care provided in each 24-hour period shall, when totaled for the entire facility, be a minimum of 2.7 hours of direct resident care for each resident.
Observations:
Based on a review of nursing time schedules and staff interview, it was determined that the facility failed to provide the minimum number of general nursing care hours to each resident in a 24 -hour period on one of 21 days reviewed.

The findings include:

A review of nursing time schedules revealed that the facility failed to provide 2.7 hours of general nursing care to each resident in a 24-hour period on the following dates:

March 10, 2019 - 2.68 hours provided

During interview on March 20, 2019, at approximately 11:50 a.m., the director of nursing confirmed the facility failed to provide minimum nursing hours on the aforementioned day.



 Plan of Correction - To be completed: 04/16/2019

Preparation and Submission of this plan of correction is required by State and Federal law. This plan of correction does not constitute an admission for purposes of general liability, professional malpractice, or any Administrative or other court proceedings.


Ridgeview will provide a minimum of 2.7 hours of direct resident care to each resident in a 24-hour period.

The RNs will be educated on the importance of maintaining acceptable minimum staffing levels as well as how to calculate the daily Nursing PPD.

The DON/ADON/Nursing Scheduler will monitor the daily Nursing PPD to ensure the Facility maintains required staffing levels and will make adjustments as necessary.

Audits of the daily Nursing PPD will be conducted by the DON/Designee for compliance 5x/week x 2 weeks, 3x/week x 2 weeks, weekly x 2 months. Results of the audits will be reported at the Facility's monthly QA meeting.
