Based on an Emergency Preparedness Survey completed on January 30, 2024, at Kinzua Healthcare and Rehabilitation Center, it was determined there were no deficiencies identified with the requirements of 42 CFR 483.73.

Based on a Medicare/Medicaid Recertification Survey completed on January 30, 2024, it was determined that Kinzua Healthcare and Rehabilitation Center was not in compliance with the following requirements of the Life Safety Code for an existing health care occupancy. Compliance with the National Fire Protection Association's Life Safety Code is required by 42 CFR 483.90(a).

This is a one-story, Type II (000), unprotected, non-combustible building, that is fully sprinklered.




 Plan of Correction:


NFPA 101 STANDARD Electrical Systems - Receptacles:This is a less serious (but not lowest level) deficiency and is isolated to the fewest number of residents, staff, or occurrences. This deficiency is one that results in minimal discomfort to the resident or has the potential (not yet realized) to negatively affect the resident's ability to achieve his/her highest functional status.
Electrical Systems - Receptacles
Power receptacles have at least one, separate, highly dependable grounding pole capable of maintaining low-contact resistance with its mating plug. In pediatric locations, receptacles in patient rooms, bathrooms, play rooms, and activity rooms, other than nurseries, are listed tamper-resistant or employ a listed cover.
If used in patient care room, ground-fault circuit interrupters (GFCI) are listed.

Observations:

Based on observation and interview, the facility failed to maintain electrical receptacles in two of over fifty rooms.

Findings include:

Observation on January 30, 2024, between 10:32 a.m. and 10:42 a.m., revealed the first floor Alzheimer unit had electrical outlets located within six feet of the sink, but were not ground fault circuit interrupter (GFCI) receptacles:
A. (10:32 a.m.) Bathroom across from resident room 549;
B. (10:42 a.m.) Bathroom in dining/recreational room.

Interview with the administrator on January 30, 2024, at 10:42 a.m., confirmed the receptacle deficiencies.




 Plan of Correction - To be completed: 03/06/2024

1. What systemic changes will be put into place to ensure that the deficiency does not recur, and how the corrective actions will be monitored?
The outlets in question were replaced on February 2, 2024. Photographs of same were emailed to the Department of Health on February 2, 2024.
2. What quality assurance program will be put into place, and the dates when corrective action will be completed?
Outcomes of the Examination for the necessity of a GFCI outlet has been added to the periodic outlet examination workorder will be reported to the QAPI committee for the next three months.
