This report is a result of an on-site complaint investigation conducted on December 13, 2022 by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, White Deer Run of York was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. |

705.3 LICENSURE Living rooms and lounges.
705.3. Living rooms and lounges.
The residential facility shall contain at least one living room or lounge for the free and informal use of clients, their families and invited guests. The facility shall maintain furnishings in a state of good repair.

Observations Based on a physical plant inspection conducted on December 13, 2022, the facility failed to maintain furnishings in a state of good repair. There were two chairs located in the activity room and one chair located in the conference room that had the material worn off the seats and the arm rests.

Plan of Correction Facility Director will ensure all furnishings are maintained in a state of good repair. The Facility Safety Officer will conduct safety walk through assessments on a monthly basis to ensure furniture is kept in good condition. Any furniture found to not be in good condition will be disposed of in the garbage and the furniture will be replaced. The two chairs that were found to be worn, which were located in the conference room and activity room, have been disposed in the trash on 12/14/22. |

(a) Exits.
(1) The residential facility shall:
(i) Ensure that stairways, hallways and exits from rooms and from the residential facility are unobstructed.

Observations Based on a physical plant inspection conducted on December 13, 2022, the facility failed to ensure that exits from the facility were unobstructed. The emergency exit located on the left side of the building was obstructed with two buckets and a pole.

Plan of Correction Facility Director will ensure all exits remain unobstructed. The Facility Safety Officer will conduct safety walk through assessments on a monthly basis to ensure exits are not blocked. If an exit is found blocked, the obstruction will be removed. The two buckets and a pole, which were obstructing an exit, have been removed and placed into proper storage as of 12/14/22. |
