INITIAL COMMENTS
 
This report is a result of an on-site licensure renewal inspection conducted on May 26 through May 27, 2011 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Mainstream Counseling, Inc. was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection.
 
Plan of Correction

705.28 (a) (1) (iii)  LICENSURE Fire safety.

705.28. Fire safety. (a) Exits. (1) The nonresidential facility shall: (iii) Maintain each ramp, interior stairway and outside steps exceeding two steps with a well-secured handrail and maintain each porch that has over an 18 inch drop with a well-secured railing.
Observations
Based on the physical plant tour and a discussion with the Facility Director, the facility failed to have a well-secured handrail on the outside steps of the main entranceway into the facility. Per regulation, the facility is to maintain each ramp, interior stairway and out side steps exceeding two steps with a well secured handrail and maintain each porch that has an over 18 inch drop with a well-secured railing.



The findings include:



A physical plant tour was conducted on May 27, 2011 at approximately 10:00 AM. At the exit interview, with the facility director and clinical staff present, it was confirmed that the facility did not have a well-secured handrail coming into the main entrance of the facility.
 
Plan of Correction
On Tuesday, May 31st, the Facility Director spoke with the owner of the physical property about the unsecured handrail which comes into the main entrance of the facility. The owner of the property is responsible for the functioning of the property. She will speak with a masonry contractor to resolve the issue. The owner of the property will make contact by phone with a contractor by June 10, 2011. The contractor will secure the handrail and address any other masonry issues with the entrance. Target date for completion will be July 30th, 2011.
